How We help

NDIS Participants

1. Choice and Control – we are able to deliver therapeutic exercise interventions: in peoples homes, in the local community, at local gyms, at local hydrotherapy centres, in our private clinic, and/or online via telehealth.

2. NDIS registered provider – we can help all participants agency-managed, plan-managed, and self-managed. We have invested time and resources in to developing a quality service, worthy of passing the NDIS audit.

3. We are community-based – Our innovative approach allows us to assist clients in all suburbs in Newcastle, the Central Coast, Hunter Valley and Port Stephens

4. Plan review reports – we can help you by completing a detailed plan review report when your plan is nearing its end, to support further reasonable and necessary therapy.

5. We are outcome focussed – we set personal goals alongside your NDIS goals and then set an individualised plan to help you achieve your goals.

6. We are consistent – where possible, we aim to assign our practitioners to participants for the long term to help them see their goals through.

Flexible Service delivery Model

Choice & Control

We give you ultimate choice and control over how you would like consultations.

We take away the stressors of travel and time by meeting a participant at their home. This mode of service delivery is great for participants with limited physical mobility, limited access to transport, or psychosocial disabilities that lead to reduced confidence in leaving the home.

We also find that our participants get great results with home programs as it allows us to show participants exercises in the environment they will complete them in when we are not there.

Home visits also provide our practitioners with a deeper understanding of the participants living environment, day to day barriers, and challenges, which then allows us to get appropriate strategies in place e.g. referral to an OT for home modifications.

If a participant has a gym membership or is looking to purchase one, we can attend the gym alongside them to optimise their program, ensure that it is safe and effective, and individualise it to help them achieve their goals. As we specialise in working with people with disabilities, we can often show our participants new ways of doing things and new exercise techniques that they may not have thought of.

Gym based training is a fantastic way for participants to meet like minded people. It also provides participants with access to specialised equipment (i.e. weights and cardio machines) that can help them to become the best version of themselves. We have great relationships with a large majority of the gyms across our wide service area – chat to us today to find out if we work with your gym!

Our Exercise Physiologists are able to complete consultations throughout a participants local community, whether it be at a local park, beach, walking trail or sporting field, we’ve got it covered! This often allows participants to access their programs outside of scheduled consultations and participate in activity more regularly. 

Community consultations are beneficial in that they allow people to move in a way that is meaningful for them, and they are a great way to promote social participation.

We are able to access local aquatic centres to deliver hydrotherapy based programs to our participants.

Hydrotherapy programs can be especially beneficial for people with neurological conditions, people who have reduced balance, people who are in an acute/sub-acute phase of rehabilitation, and of course, people who enjoy water based activity! 

Our fully equipped clinic at: Suite 1, 316 Maitland Road Mayfield, gives participants access to all of the equipment they could need to improve their health and assist them towards their goals.

The clinic always has a great, supportive environment – participants get all the benefits of a gym and equipment, without the crowds. The clinic is accessible, with lots of on-street parking nearby; we also have accessible shared amenities on-site.

Completing clinic based consultations saves participants funds as they won’t be charged for travel.

We are well versed at being able to develop and deliver quality online exercise programs via Microsoft Teams.

This allows participants to access, and stay on track with their exercises no matter where they are. We find that certain participants love engaging via telehealth, especially if they are managing social phobias, psychosocial disabilities, or just don’t have the confidence to be seen in person yet.

We have had some fantastic outcomes where people begin with online programs and then increase their social participation over time as their confidence grows.

After being on work cover for a year and a half I finally had surgery to which my recovery and time on WC had been super draining mentally, physically and emotionally. My confidence was at a all time low and after being in pain for so long I couldn't tell what was actual pain and what was weakness. Luke has been amazing, his down to earth approach eased my mind and he was caring enough to truly listen and guide me on my recovery. He has looked at my injury from all views and has helped me build my confidence back up that I'm now going to the gym strength training 4-5 times a week. He has given me so much knowledge about my body and mind and I couldn't thank him enough for all the support over the past few months. I had an excellent experience with Jason from Uplift. I'd previously seen a physio who provided me with general exercises. Jason at Uplift went further by tailoring my rehab to my lifestyle. For example, after a short discussion he found out I was active in the water and incorporated swimming strokes to strengthen my shoulder as part of my rehab. This personal and understanding touch helped fix my shoulder faster and didn't require me to change my lifestyle by joining a gym or lifting weights. Thanks very much to the team at Uplift.

FAQs

Absolutely! We have a great mix of male and female Exercise Physiologists of varying ages and levels of experience. When we receive a referral we review it and allocate the participant to the most appropriate practitioner. Please make sure to specify any preferences upon referral and we will do our best to accommodate.

We have great coverage. We deliver NDIS Exercise Physiology services to participants across all suburbs in Newcastle, the Central Coast, the Hunter Valley, and Port Stephens.

We can also consult with NDIS participants Australia-wide, via telehealth.

For clinic based consultations you can see us at: 1/316 Maitland Road, Mayfield, 2304.

Absolutely! We have great relationships with the majority of gyms and hydrotherapy centres within our service area. We cover our own entry, so long as the participant covers their own entry.

No unfortunately gym and pool memberships are a cost for the participant to manage.

We are happy to help people cover the costs of basic exercise equipment if we deem it helpful for them to achieve their NDIS goals. Unfortunately, we cannot pay for extensive exercise equipment.

Yes we charge travel as follows:
– If the session takes place within 30 minutes drive from Mayfield Clinic, we charge 15 minutes.
– If the session takes place greater than 30 minutes drive from Mayfield Clinic, we cap travel at 30 minutes.

If you want to minimise travel costs, you can discuss session length, clinic visits, locations, online sessions with your Exercise Physiologist.

The initial assessment is where the participant and Exercise Physiologist get to meet for the first time. The purpose of the initial assessment is for the participant and Exercise Physiologist to get to know each other, discuss goals, discuss health history, assess current status (questionnaires and basic physical assessments), and most importantly collaborate together to develop an individualised exercise plan. The initial assessment is usually more talk based, rather than exercise based – the majority of exercises begin after the initial assessment.

At the end of the assessment, once both parties agree on a plan, a Service Agreement is completed which formalises the agreement between the participant and Uplift around the delivery of Exercise Physiology services.

No two programs look the same, so we really can’t know this until after the initial assessment.

The reason for this is that we take an individualised approach, not a ‘cookie cutter’ approach. Some of the factors that determine what a program look like include: participant goals, participant exercise preferences and likes, disability/medical conditions, safety, and current evidence around what is effective.

Programs can be delivered at home, in the gym, at a hydrotherapy centre, online or in the community (or a combination of multiple options).

This is different for everyone.

In an ideal world, we help people to be able to self-manage their exercises and they can manage independently into the future. For some, this may be achieved in just a few consultations.

For others with more complex needs, and who may not be able to self-manage consultations may be more regular and continue for a longer period of time.

We usually focus on making a short to medium term plan for the the participants current NDIS plan period and review periodically, and when the plan review is approaching to determine future needs.

Yes! We have been in operation since 2019 and the longest wait list we have ever had is two weeks, and even that was only for a short period of time. We have managed to hire and retain quality team members which has allowed us to meet increasing demand and avoid wait times.
Absolutely! At Uplift we pride ourselves on our communication so that you always know how your participant is progressing, or if there are barriers that need to be addressed. Our communication may be in the form of an email, call or text either side of an appointment, or a plan review report in the leadup to a plan review.

WE HELP PARTICIPANTS WITH ALL DISABILITES INCLUDING (BUT NOT LIMITED TO):
Physical Disabilities: Cerebral Palsy, Paraplegia, Cystic Fibrosis, Epilepsy, Multiple Sclerosis, Muscular Dystrophy, Amputation, Chronic Pain, Acquired Brain Injuries, Spinal Cord Injuries, Spina Bifida, Parkinson’s Disease, Stroke, Hearing/Vision Impairment.
Psychosocial Disabilities: Schizophrenia, Bipolar Disorder, Eating Disorders, Depression, Anxiety, PTSD.
Intellectual Disabilities: Down Syndrome, Williams Syndrome, Prader Willi, Autism, ADHD, ADD, Fragile X Syndrome, Developmental Delay, Foetal Alcohol Spectrum Disorder.

This is completely dependant on an individuals situation. 

At a minimum we recommend 3.5 hours which would allow us to travel, complete an assessment and complete a report which could then be taken to a plan review to discuss with your NDIS Planner.

Some people who already have funding for Exercise Physiology, may wish to trial the service before fully committing -10 hours of funding would allow for this.

Other people are committed to Exercise Physiology and may factor in up to 2-3 hours per week to help them towards their goals.

It is all very individualised and we certainly respect choice and control.

Exercise Physiology is a Capacity Building Support.

You can use funding from either of the two Capacity Building categories to access Exercise Physiology:
– Improved Daily Living (CB Daily Activity).
– Improved Health & Wellbeing (CB Health and Wellbeing).

No, unfortunately we cannot accept Core funding for Exercise Physiology.

A lot less than you may think.

Exercise Physiologists and Physiotherapists both need to complete at least four years of university training to become qualified. They are both classified as allied health professionals. They are both trained in prescribing exercise, education and behaviour change interventions to help participants achieve their goals. 

Physiotherapists are able to provide manual therapy and diagnose, however as we know diagnosis is usually irrelevant in the NDIS as diagnosis is established before someone qualifies for the NDIS, and benefits of manual therapy for chronic conditions and disabilities is questionable.

As Exercise Physiologists spend ALL of their time focussing on delivering exercise, education and behaviour change interventions (and no diagnosis/manual therapy), it is safe to say that they are the specialists in that area, and there is no profession in Australia more highly trained when it comes to that.

Additionally, the current NDIS rate for Exercise Physiology is $166.99, while the current NDIS rate for Physiotherapy is $193.99. This means participants choosing Exercise Physiology will get ~16% more value, out of choosing Exercise Physiology over Physiotherapy.

Note: within both professions practitioners have sub-specialisations – it would be ideal to link with a practitioner that has a sub-specialisation in the disability/condition that you are seeking assistance with.

Absolutely!

The quickest and easiest way to start Exercise Physiology with Uplift is to fill out our online referral form [LINK TO FORM]. Once this form is filled out, it will be received and reviewed by the team manager who can immediately begin processing the referral into our system. Once processed into the system, it is allocated to the most appropriate Exercise Physiologist (considering geography, complexity, gender requests etc.) and the Exercise Physiologist will be in contact to book an initial assessment at an agreed time, date and location.
